Instructions for applying for assistance through MHMR

Before you apply for assistance through the MHMR 
you will need to gather the following documents:

	Medicaid Card
	Current Immunization Record
	Social Security Card
	Birth Certificate
	Proof of Income (paycheck stubs, W2s or other proof)
	Official diagnosis (from doctor or school diagnostician)
